Short-term disability documentation support and form completion (when clinically appropriate)

By using our telehealth services, you understand and agree that:
Care may be delivered via video, phone, or secure messaging depending on visit type
Telehealth has limitations (e.g., inability to perform hands-on exams)
You are responsible for having adequate internet/phone access and a private setting
If your provider determines your condition requires in-person evaluation, labs, imaging, specialist evaluation, or emergency care, you may be referred accordingly
You agree to:
Provide accurate, complete health and identity information
Upload/submit required forms and supporting documents in a timely manner
Respond promptly to requests for clarification
Use the Site and services lawfully and respectfully
If information is incomplete or inaccurate, services (including documentation) may be delayed, limited, or declined.
For medical forms, letters, and documentation services:
Standard turnaround is 5–7 business days after your evaluation and receipt of all required forms and supporting information
Rush service may be available for an additional cost depending on complexity and scheduling availability
Some forms require more than one visit and/or supporting records, objective findings, or labs. If so, you will be informed
Important: We will only complete forms that are truthful, clinically appropriate, and consistent with the evaluation and documentation standards.
Short-term disability requests are often reviewed by employers, HR departments, and insurance carriers. By requesting short-term disability documentation support, you understand and agree that:
Completion of forms does not guarantee approval of benefits, leave, accommodations, job protection, or payment
Employers/insurers may require additional documentation, diagnostic testing, records, specialist notes, or in-person evaluation
We cannot and will not alter clinical findings to meet an outcome or timeline
We do not complete or sign documentation that is inaccurate, incomplete, misleading, or not supported by clinical assessment
Final decisions regarding eligibility and benefits are made solely by the employer/insurer and are outside our control
Fees (including rush fees) are disclosed at or before the time of purchase/booking. By booking or purchasing, you authorize payment using the method you provide.
Refunds, if any, are handled according to the policy communicated at booking and may depend on:
Whether services were delivered
Whether a visit was missed or canceled
Whether work (documentation/chart review) has already begun
Appointment changes, late arrivals, cancellations, and no-shows may be subject to fees as disclosed at booking. If you cannot attend, notify us as soon as possible.
When clinically appropriate, prescriptions may be provided. Prescribing is based on:
Medical history, assessment, and clinical judgment
Applicable laws and professional standards
Patient safety considerations
We may decline to prescribe medications if unsafe, inappropriate, or legally restricted in the telehealth setting.
